urlcassifier2.sqlite from Firefox isn't the only potential culprit. Either Safari or Flash Player on my computer was causing about 3 0xe0030005 errors every 5 minutes. Reinstalling both (just to be safe) seems to have fixed the problem for me. If you're getting this error not too often, back up your files and see if you can narrow down what applications are running every time you get the error (and you get the error every time they are running).

I seem to have the same problems, many spinning beachballs some for a few seconds and others for over an hour or the system just crashes. Ive tried resolving the problem by reformatting the H/D, then installing new ram but this did not resolved the problems. Have now installed a new H/D but still have the same problems. I have not installed any files from the old H/D. Fans are working and there is no excessive heat. Any ideas?

It's been several weeks following the reformat-the-HD fix and it's still working great. No more “kernel: disk1s2: 0xe0030005 (UNDEFINED)” messages in the log. No excuse to buy that WD 320 GB Scorpion.

Maybe I should go check the logs again just to makes sure.... ;-)

Re-installed Mac OS X and TechTool isn't reporting any errors. No more “kernel: disk1s2: 0xe0030005 (UNDEFINED)” messages in the log either. I'm still worried the drive is failing tho'.

Got the same error on my MacBook (Intel): "kernel: disk1s2: 0xe0030005 (UNDEFINED)" -- the system is quite slow and unstable, some apps won't startup, spinning beachball, etc.

On the one hand...

SMART status: verified
fsck says everything is ok
Disk Utility says everything is ok

However...

TechTool Deluxe (available for free if you have AppleCare -- https://support.apple.com/techtooldeluxe/ ) hangs (or locks up) when attempting to do any of the "Dirctory Scan", "Surface Scan" or "Volume Structure" tests.

Attempting to TimeMachine backup fails with an error.

Making a backup using Backup.app takes an inordinately long time (1.5 days for a 160GB disk!), but at least it appears to have succeeded.

So...

Zapped PRAM and reformatted the drive and now TechTool reports everything is ok, but I haven't tried installing anything on it, so who knows.

Looks like time to get a new drive....

(Hmm, those Western Digital 320 GB Scorpions for $100 sure look tasty!)

Also had the problem today ...

MB is running Vista for a few weeks now on the second partition and had leopard NOT running for quite some time (since the 10.5.2 update). Then rebooted into OSX to get a few files without problems.

While working, everything started to slow down until the system hung. No more reboot possible. The startup DVD can be used to check the drive - which showed up in DiskUtil only after a manual fdisk in a terminal window. No errors reported. Then did a reinstall. But the system does not come up after the reinstall but hangs with the UNDEFINED error. Vista still boots fine but also shows signs of slowdown. I also hear the similar HD grinding noise as with the OSX boot - but it is inconclusive.

Not sure what to do next.

I hate to double post but after doing a very tedious file copying in single-user mode I was able to save all of my vital info.

By the grace of Bit Torrent I was able to acquire a copy of leopard to reinstall OS X.

I am *VERY* skeptical that this error is strictly a hard drive failure error. I'm running 100% normal again with all my programs, settings, and files intact by just doing a basic reinstall.

I just got this on my macbook today (it's less than 11mo old). It happened this morning when I opened itunes and the laptop slowed to a grinding halt. I did a hard reboot and it has never rebooted again into OS X.

Performed all the "fixes" mentioned here to no avail.

I highly doubt this is a hardware (read: bad hard drive) issue for me. My Vista partition on the same laptop works perfectly, no errors, no problems.

To the people that are complaining of having replaced multiple HD's: Are you doing backup restores onto these new HDs? If this were a software issue, wouldn't that explain the repetitive "bad HDs"?

Worst part is that I'm stuck in Peru for another month without my discs, spare hardware, and more importantly...an apple store.

I've had word for word the exact same problem as Jeremy Dickens. This is the third time I've had this problem and solved it with a new hard drive. I'm not convinced that this is merely a hard drive issue, if 3 hard drives failed from the same computer...clearly there is something else wrong.
